Tesla Reveals Details of 17 Robotaxi Crashes: Mostly Rear-Ended, Including Two Remote Operation Errors

Tesla has submitted and unsealed the full original content of all 17 Autopilot collision reports to the U.S. National Highway Traffic Safety Administration (NHTSA), disclosing for the first time the specific details of each incident. These accidents occurred between July 2025 and March 2026, all in Austin, involving 2026 Model Y vehicles equipped with safety drivers, and all took place while the Full Self-Driving (FSD) feature was engaged. According to the reports, 13 incidents resulted only in property damage, two involved no reported injuries, one caused a minor injury to the safety driver that did not require hospitalization, and another led to a minor injury requiring hospital treatment. Most collisions occurred when Tesla vehicles were rear-ended by other vehicles while stationary—at red lights, stop signs, or within traffic flow. The striking vehicles included passenger cars, trucks, three-wheelers, buses, and SUVs, which collided with or scraped the Tesla from behind or the side. Additionally, two accidents were caused by errors made by remote operators: in July 2025, a remote operator took control and struck a metal barrier at 8 mph; in January 2026, another remote operator hit a construction zone barricade at 9 mph. Several other incidents involved FSD system issues with spatial perception of static obstacles—for example, colliding with a metal chain during an unprotected left turn, scraping a trailer gooseneck hitch, or hitting a utility pole or curb while reversing. The most severe incident occurred in a dedicated right-turn lane: the Tesla was moving slowly at 2 mph and continuously yielding, yet was rear-ended by an SUV, resulting in the safety driver’s injury and subsequent hospitalization. Previously, Tesla had redacted all collision report details entirely, citing concerns that they “may contain confidential business information.” This unsealing now provides the public with its first comprehensive view of real-world accident scenarios during Tesla’s Robotaxi testing phase.
