On the evening of June 19, 2024, a 44-year-old man named Butler was driving a 2025 Tesla Model 3 in a residential area with a speed limit of 48 km/h (30 mph) when he activated the Full Self-Driving (FSD) feature. The vehicle veered off the road and crashed into a house, resulting in the death of a 76-year-old resident who sustained fatal injuries. On July 15, the U.S. National Transportation Safety Board (NTSB) released a preliminary investigation report concluding that the crash was not caused by an FSD system malfunction and that the primary responsibility lay with driver error. Following the incident, Butler claimed he had lost consciousness while using FSD and attempted to blame the crash on system failure. However, police investigators discovered his Google search history included queries such as “2026 Tesla FSD not aggressive enough” and “Tesla FSD too conservative,” indicating dissatisfaction with the system’s cautious speed limits and suggesting he had deliberately accelerated the vehicle. Tesla stated that in residential zones, FSD operates at low cruising speeds, but if the driver presses the accelerator pedal deeply, the system prioritizes manual input and disengages autonomous control. Vehicle data showed the car was traveling at approximately 117 km/h (73 mph) at the time of impact. Tesla CEO Elon Musk publicly emphasized that the high-speed collision resulted from human error, with no software malfunction involved. The NTSB report also noted that the National Highway Traffic Safety Administration (NHTSA) will conduct a thorough review of the vehicle’s sensor data and driver monitoring system records, with a final comprehensive report to follow. All current evidence has ruled out the possibility of an FSD software fault or mechanical vehicle failure. The driver has been arrested and faces criminal charges of vehicular manslaughter. The victim’s family has filed a civil lawsuit against both the driver and Tesla, alleging deficiencies in the vehicle’s warning mechanisms and driver monitoring systems.
