On August 31, the regulator of the UK’s National Health Service (NHS) issued a warning that AI-powered medical documentation tools may misidentify medication and disease names when transcribing conversations between patients and doctors, posing a threat to patient safety. In one case, a female patient was mistakenly recorded as having “demyelination”—a condition potentially linked to multiple sclerosis—due to an AI-generated summary of her consultation with a doctor, causing her severe distress. Healthwatch England, the national patient advocacy body, disclosed this incident along with other similar errors, warning that if such mistakes go undetected, they could be permanently added to patients’ medical records and adversely affect future care. Healthwatch stated it has received reports from multiple patients who discovered inaccuracies in AI-generated notes that healthcare professionals failed to notice. Currently, general practitioners and hospital doctors across England use 27 different AI clinical documentation tools. The UK government aims to deploy these tools to reduce administrative burdens on healthcare staff, thereby freeing up more time for direct patient care, and considers them a key component of the NHS’s digital transformation. However, Healthwatch pointed out that the Medicines and Healthcare products Regulatory Agency (MHRA) has not yet classified AI clinical documentation tools as medical devices, and there is no nationwide regulatory framework in place across England. Government ministers have been informed that if AI tools lead to clinical errors, the NHS and healthcare professionals could face legal action.
